Biography

Peter Donnan is Professor of Epidemiology and Biostatistics in the Medical School Division of Population Health Sciences within the Medical Research Institute, University of Dundee. He has 20 years’ experience in the design and analysis of Health Services and Population Research observational studies as well as randomised controlled trials of drugs (recently steroids in Bell’s Palsy, NEJM, 2007) as well as public health interventions (screening for breast cancer, Lancet, 1990). He leads the Dundee Epidemiology and Biostatistics Unit (DEBU) and co-leads the Tayside Clinical Trials Unit (TCTU) which has UKCRC registration. He has also carried out extensive population record-linkage research on drug safety, adherence and management of long term conditions using Tayside’s unique prescribing population databases and is the author of over 150 publications (h-index 33, m-index 1.25), as well as an investigator on research projects to a total of over £26 million. He has developed a number of prognostic algorithms, most notably predicting Coronary Heart Disease (CHD) in people with type 2 diabetes (Diabetes Care, 2006), and Chief Scientist Office funded algorithm to predict risk of emergency admissions in primary care (PEONY) with a view to developing cost-effective interventions for long term conditions (Archives Internal Medicine, 2008). A recent NIHR HTA-funded study has developed aids to clinical decision making with abnormal liver function tests in primary care. In 2004, Prof. Donnan was invited to become a member of the New Drugs Committee of the Scottish Medicines Consortium which seeks to advise Area and Drug Therapeutics Committees across Scotland regarding the cost-effectiveness of new drugs.

Current Research

Epidemiology and Management of Long Term Conditions

This is a major area of research and coincides with a main concern of Population Health Sciences and recent International and UK health policy. I lead a team of four research fellows, all of whom are also supervised by me for their PhD degrees. In particular, I have an international reputation in the area of development and validation of clinical predictions aids, having published in Diabetes Care the first population-derived algorithm to predict CHD in people with type 2 diabetes. Diabetes UK and Pfizer have extended funding to develop a similar algorithm for stroke.

I have also developed an algorithm to predict emergency hospital admissions in people aged over 40 years which is again unique. This was published in Archives of Internal Medicine and CSO provided further funding to extend the model with keen interest from the Scottish Executive. NHS Tayside and Grampian have now adopted this algorithm in their programme to manage long term conditions. This has been a continuous theme throughout my career having developed a prediction tool for adverse outcomes in Hodgkin’s disease from my MSc thesis, which continues to be used today. All of these studies have relied on my advanced skills in epidemiological analysis of observational data.

Having derived probabilities of outcomes using observational data, these can be incorporated into decision analyses which aim to optimise clinical outcomes, resources and patient-preferences. I obtained funding from Department of Health HTA to produce a decision support tool for use in primary care for patients with abnormal liver function tests using the record-linked Tayside database of liver disease; Evaluation of Liver Disease in Tayside (ELDIT).  The design of such tools will provide a template for clinical aids in other long term conditions where management decisions are crucial to patient care and must be cost-effective.

Cancer with improved treatment is continuing to be seen as a long term condition Outcomes in cancer incorporates the analysis of time to event data (survival analysis), especially dealing with multiple events and frailty. I am also interested in screening for cancer having previously analysed the Edinburgh randomised controlled trial of screening for breast cancer and currently assessing PSA testing. I have completed a PCRF assessment of co-morbidity in colorectal cancer and in a CRUK study used multi-level models to consider time from first presentation of colorectal and breast cancer to treatment in collaboration with colleagues in Aberdeen and Edinburgh.

I also have an interest in spatial clustering and statistically rigourous methods for testing for clusters of multiple sclerosis, such as spatial scan statistics, which has also just been published in the internationally recognised journal, Multiple Sclerosis.

Randomised Controlled Trials

The design and analysis of randomised controlled trials is a major interest having been involved in the first large RCT of breast cancer screening in the UK, RCT of anxiety treatment, smoking cessation, early discharge following surgery for breast cancer and recently, a Scottish-wide factorial trial of Bell’s Palsy. I am currently co-director of the Tayside Clinical Trials Unit (TCTU) which has UKCRN registration. I have been a member of several DMECs for UK and international trials. A particular interest is the design and analysis of randomised controlled trials where the unit of randomisation is the general practice, which has been utilised in recent applications to GSK and NIH, USA. The organisation of primary care into practices within populations leads naturally to the use of multi-level modelling in such studies. I am currently providing statistical input in design and analysis of several TCTU trials.

Pharmacoepidemiology

Previously I was the Research Manager and Senior Medical Statistician of the internationally recognised Medicines Monitoring Unit (MEMO), and have developed a new unit devoted to biostatistics, Dundee Epidemiology and Biostatistics Unit (DEBU). As head of DEBU I   supervise three other statisticians/ epidemiologists. As well as leading the design and implemention of studies utilising the whole population of Tayside (~420,000) I was responsible for improving and maintaining internationally recognised epidemiologically rigourous studies. The record-linkage of prescription data to morbidity and mortality facilitates the creation of anonymised datasets that can be used in adherence studies and population retrospective cohort studies of drug safety, as well as pharmaco-economic studies. I was principal author of a study which for the first time demonstrated the link between antibiotic resistance and antibiotic prescribing which was published in the BMJ. I was principal investigator for a National Osteoporosis Society study of the risk of non-traumatic fracture in relation to corticosteroids, which has recently been published. I developed methods to measure adherence to medication from drug prescription databases, which led to numerous publications and recognition of my international expertise in this area from invitations to be key speaker at international conferences. Recent work has demonstrated the link between genotype and response to statins in a diabetic population. I am also a member of the workstream of pharmacovigilance in the Scottish Health Informatics Programme (SHIP) utilising routine datasets of community prescribing.

Epidemiological Methodology

Observational analyses such as cohort and case-control have been a constant interest throughout my career. My expertise in this area has been recognised by invitations to write reviews and I have also implemented novel designs such as case-crossover and the use of propensity scores to reduce confounding in observational studies. Case-crossover design has been implemented in a CSO Research Training Fellowship (warfarin related hospitalisation for bleeds) and a proposed study with Amgen, USA on anaemia as a trigger for CHD events in type 2 diabetes. The use of Bayesian methods to model observational data, especially with a hierarchical structure, are underutilised in health services research and are major areas of interest.
